The government provide funding for primary healthcare in New Zealand. The amount of money a medical centre gets depends on the age, gender, ethnicity, and address of the people using their services.  To get this information, medical centres ask patients to complete enrolment forms. This is why a medical centre will ask if you are enrolled or want to enrol when you visit.
